Fortify Optimal Health, LLC ("Fortify," "we," "us," or "our") respects your privacy and is committed to protecting the personal information you provide through the Fortify Optimal Health mobile application (the "App").
This Privacy Policy explains what information we collect through the App, how we use and disclose that information, how long we retain it, and the choices available to you.
The App is intended exclusively for adults who are 18 years of age or older. You must be at least 18 years old to create an account or use the App. By creating an account or using the App, you represent and warrant that you are at least 18 years of age.

This Privacy Policy applies to personal information collected through the Fortify Optimal Health App and related App services.
The App is designed as a self-monitoring, organizational, educational, and motivational tool for adults. The App allows users to track information such as weight, nutrition, exercise, injection dates, and mindset-related information.
The App does not provide medical diagnosis, medical treatment, emergency services, or individualized medical advice. Information generated or displayed by the App should not be used as a substitute for evaluation or advice from a qualified healthcare professional.
The App is not intended for, offered to, or authorized for use by individuals under 18 years of age.
Fortify Optimal Health, LLC also provides healthcare services. However, use of the App should not be assumed to constitute medical treatment or communication with a healthcare provider.
Information entered into the App is not reviewed or monitored by a healthcare professional. Entering information into the App does not mean that a Fortify healthcare provider has received, reviewed, or acted upon that information.
Do not use the App to communicate an emergency, urgent medical concern, medication reaction, significant change in symptoms, or other time-sensitive health information. If you believe you are experiencing a medical emergency, seek appropriate emergency medical care.
Certain information maintained by Fortify in connection with healthcare services may be subject to separate privacy requirements, including Fortify's Notice of Privacy Practices where applicable. This Privacy Policy governs information collected through the App unless another applicable privacy notice or law governs the information.

AI-generated information may be incomplete, inaccurate, or inappropriate for your individual circumstances. AI-generated information is provided for informational and organizational purposes and should not be considered medical or nutritional advice, diagnosis, or treatment.

You may request permanent deletion of your App account and associated personal information.

Account deletion is intended to remove the account and personal information associated with that App account, subject to the limited retention circumstances described in this Privacy Policy.

Deleting your Fortify account does not necessarily cancel an active Apple App Store or Google Play subscription.
Subscriptions purchased through Apple or Google are managed by those companies and should be canceled through the applicable Apple or Google subscription settings to prevent future renewal charges.

Use of the Fortify Optimal Health App does not, by itself, create a new healthcare provider-patient relationship.
If you separately receive healthcare services from Fortify Optimal Health, information generated in connection with those healthcare services may be governed by additional privacy notices and applicable healthcare privacy laws.
Information voluntarily entered into tracking features within the App will not be assumed to have been reviewed by your healthcare provider or incorporated into your designated medical record unless specifically communicated or documented through the appropriate healthcare process.
Do not rely on App tracking as a means of notifying a healthcare provider about symptoms, medication reactions, emergencies, or other matters requiring medical attention.
The Fortify Optimal Health App is intended exclusively for individuals who are 18 years of age or older.
You must be at least 18 years old to create an account, subscribe to, access, or use the App. By creating an account, subscribing, or using the App, you represent and warrant that you are at least 18 years of age.
Individuals under 18 are not authorized to create an account or use the App, even with the consent of a parent or legal guardian.
Fortify does not knowingly collect personal information through the App from individuals under 18 years of age.
If Fortify becomes aware that an individual under 18 has created or used an account, Fortify may immediately suspend or terminate access to the account and take reasonable steps to delete personal information associated with the account, subject to applicable legal obligations.
